Privacy statement

HEALTH INFORMATION PRIVACY STATEMENT:

Privacy and confidentiality of your clinical information is important to us and will be fully respected.  This statement sets out why your information is displayed, how that information will be used and how you can obtain a copy and / or restrict the information reflected.

 

PURPOSE:

HealthOne is a secure electronic health record displaying health information from Primary and Community Health Care Providers.  Authorised Healthcare Providers such as GPs, Community Nurses, Pharmacists and Hospital Doctors and Nurses can access information displayed in HealthOne.

 

CONFIDENTIALITY AND INFORMATION SHARING:

Your privacy and the confidentiality of information is important to us;

  • We display relevant information from your clinical care that allows those directly involved in your care to share and access your healthcare information such as medication, allergies, health warnings, hospital and nursing information such as test results.

Your clinical health information does not have to be shared, however, withholding it may affect the quality of care you receive (https://healthone.org.nz/privacy-security#choice).  Please speak to your Healthcare Provider or call 0508 867 872 if you have any concerns.

  • You have the right to know where your clinical information is kept, who has access rights, if the system has audit log capability, and who has viewed your clinical information.

  • Your clinical information will be kept securely to prevent unauthorised accesses and all accesses are proactively audited on a scheduled fortnightly basis.

 

INFORMATION COLLECTED AND YOUR RIGHT TO ACCESS AND CORRECT:

We are required to safely and securely store a copy of the clinical information relevant for your treatment and care, as shared by your healthcare providers.  We cannot alter or amend this shared information.  Should you wish to amend your information, please speak to your Healthcare Provider directly, alternatively contact Health NZ Te Whatu Ora -Patient Information Office at (03) 378 6907 or 0800 855 066 for queries relating to hospital sourced information.

  • You have the right to request a copy of your clinical record displayed in HealthOne.You don’t have to explain why you are requesting the information but will be required to provide proof of your identity.

  • You have the right to request a report outlining who has accessed your HealthOne record.As above, you do not have to explain why you are requesting the information but will be required to provide proof of your identity.

It is also your right to opt off HealthOne or limit what Healthcare Providers can see when dealing directly in your health care.  If you would like to opt-off, you can email privacy@healthone.health.nz, call the HealthOne

USE OF YOUR HEALTH INFORMATION:

General Practice Doctors, Nurses, Nurse Practitioners, Pharmacists, Pharmacy Technicians and Allied Health Professionals along with Secondary Care clinicians working in hospitals throughout the South Island have the authorisation to access your HealthOne Clinical information when dealing directly in your care.

RESEARCH:

HealthOne clinical information is not used for health research, however – 

•       Your clinical information displayed in HealthOne may be accessed in research if you have given specific consent for the researchers to access your clinical information for a specific study and/or;  • You have consented to a specific research project approved by an ethics committee.

 

COMPLAINTS:

We encourage you to contact us if you are not happy with the way your health information is collected or used. Talk to your Healthcare Provider in the first instance.
